Collaborate
verb | col·lab·o·rate | kə-ˈla-bə-ˌrāt

to work with another person or group in order to achieve something

Collaborative Health is an emerging field, where practitioners from different professions work seamlessly together to provide patients with healthcare that combines their expertise. This area of healthcare recognizes the value of bringing together the skills and knowledge of diverse professions in order to improve patient care, in an integrative and cooperative way. Most importantly, collaborative care aligns with the values and needs of the patient.

At the Centre for Collaborative Health, we are passionate about serving our patients through this dynamic and innovative approach. We are thrilled to bring a collaborative care practice to the Oakville community and help shape a healthier future.

Collaborative care practice is a desired and necessary part of health care delivery in Canada and vital in quality, patient centered care.
